Resident driven solutions to health concerns and social determinants of health

Gail Zandee, MSN, from Calvin Initiatives that address SDoH, will highlight the results of a recent community assessment in Burton Heights neighborhood including a description of the social determinants of health as identified by residents. It will also include an example of how knowledge of social determinants of health is being used to promote positive health outcomes in the area of mental health.
