Participating Plans

In network coverage

To ensure that you receive the highest level of coverage from your insurance plan when you visit Calvin Health Services, we recommend changing your primary care provider (PCP) to Laura Champion, MD. By selecting Dr. Champion as your PCP, more of your care is paid by your insurance company. This means less out-of-pocket costs for you, especially for vaccines and preventative care.

Click on your plan name below to see how you can change your Primary Care Provider:

  • (only if Dr. Champion is listed as your primary care provider)
For all other plans, contact your insurance carrier to see how changing your PCP would affect your coverage.
