Student Concerns

The Student Concern form is available to any student, staff, or faculty to use in requesting support for a student of concern at 黄大仙高手论坛. In addition to behavioral or wellness challenges, the form can also be used for concerns related to academic advising, academic performance, class attendance (roster and attendance checks), and general safety. Family members without access to the form can submit a concern by contacting the Dean of Students office or Coordinated Care team directly (see contact information below).

Information submitted via the Student Concern form will be reviewed during business hours by a member of the Dean of Students office, a member of the Coordinated Care team, or another member of the听CARE Team. Concerns will be routed to the appropriate individual or team for follow-up.
